You can enhance your request form by using Link to database fields as options for single or multiple select answers. This allows respondents to select predefined entries from your database, ensuring data consistency and simplifying data management.

Link to database custom fields function in request forms just like other custom fields. Account request forms can utilize only account-level Link to database custom fields, which you create the same way as other account custom fields. Space request forms, on the other hand, can use both account and space-level Link to database custom fields.

Link to database custom fields are not accessible in request forms on the mobile app.
To Use Link to Database Fields in a Request Form:

Start by creating a request form.

Choose a single answer or multiple answers as the question type based on your needs 1.

Click Use custom fields as option 2.

In the pop-up window, select the Link to database 3 custom field.

Once selected, all options from the Link to database custom fields will automatically populate for respondents to choose 4.

You can use Link to database fields for conditional follow-up questions in your request form. They function just like other conditional logic options in request forms.

When you use Link to database fields as options, the available choices are displayed based on their Name field within the database.

To map the answers, click the Add answer to 5 button in the lower right corner of the question box.

Ensure only the relevant Link to database 6 fields are enabled for mapping responses.
You can't make a request form external if it includes Link to database fields. If you attempt to share such a form, a warning message will prompt you to disable the public link or remove all Link to database fields.
Warning
You can’t publicly share forms with Link to database fields, because people outside your organization don't have access to these databases.
After the request form with a Link to database fields is published, the submitter's view of the form will display single-choice and multiple-choice options clearly. Respondents can then select the appropriate answers, ensuring the data is captured accurately based on the predefined fields in the database.
