Conditional Follow-up Questions in Request Forms
Use branching in request forms to control what happens next based on a user’s answer. Add branching to Single answer, Multiple answer, or Importance questions to show follow-up questions, send users to another page, or submit the form automatically.

Space admins can manage request forms in their spaces. Account admins and owners can manage account-level request forms. On Legacy Enterprise, Pinnacle and Apex accounts, this right can be revoked from account admins.
You can create dynamic request forms that include different questions and change the attributes of the task or project they create based on the answers given.
When the form respondent selects a specific Single answer or Importance option you can choose to:

Add a follow-up question to get more information

Redirect them to the next page of the form to a different set of questions

Complete and submit the form

Navigate to the request form builder for the relevant form.

Add a Single answer, Mutiple answer or Importance question.

Add all the necessary response options.

Click the branching 1 icon.

To add a question: Hover over Add follow-up question 2 and select Paragraph, then fill out the added question.

To redirect to a different page: Hover over Redirect 3 and select an existing page if you've already created more than one or choose Go to a new page 4 to create a new one.

To complete the form: Hover over Redirect and select Submit form 5 option. If users select the corresponding answer, they'll see the Submit button even if there are more pages in the form.

Repeat Steps 2-5 until you’ve added all necessary pages and questions. Click the + Action button to add other conditional branching actions to the same answer option if necessary (i.e., creating an approval). When you’re finished making changes to your form, click Publish or Save.
Note
For redirects to work correctly, there shouldn’t be more than one redirect per page.
You can add multiple follow-up questions to Single answer, Multiple answer, and Importance questions.
Single answer
Use Single answer when a person should select one option. Add follow-up questions if you want them to answer additional questions based on the option they choose.
Example question: How would you rate us?
Answer options:

Follow-up question: What did you like most?

This setup helps you collect more details based on the selected answer.
Multiple answer
Use Multiple answer when you want respondents to select more than one option. Add follow-up questions if you want them to answer a relevant set of additional questions for each option they choose.
Example question: Which parts of your experience would you like to give feedback on?

This setup lets you collect detailed feedback for each selected area without showing unnecessary questions.
Importance
Use Importance when you want people to indicate urgency or priority. Add follow-up questions if you want them to answer additional questions based on the option they select.
Example question: How important was response time to you?
Answer options:

This setup helps you understand priorities and collect more context.
Note

At the moment, if you make the follow-up question mandatory, it won't be displayed to users who submit the form via Wrike's mobile apps.

You can add an unlimited number of follow-up questions to Single answer, Multiple answer or Importance type of questions.
