Creating Multipage Request Forms

TL;DR

Multi-page forms help organize questions, improve clarity, and enable conditional follow-up questions that redirect users to specific pages. In the form builder, you can add a page from the bottom of the form or via conditional redirect options, delete a page from its three-dot menu, and reorder or merge pages using the same menu.

Overview

Space admins can create request forms in their spaces. Account admins and owners can create account-level request forms. On Legacy Enterprise , Pinnacle and Apex accounts, this right can be revoked from account admins.

Create request forms with multiple pages to:

  • Break up questions on your form by section or add clarity.

  • Add follow-up questions. Set up your form so that if a user selects a specific answer to a question, they're redirected to a specific page on the form.

Add a Page

  1. Navigate to the request form builder for the relevant form.

  2. Scroll to the bottom of the form.

  3. Click the + button under Add a question.

  4. Select New page 1.

Alternatively, when creating a Single answer or Importance question use one of the conditional form options and Redirect to select Go to a new page.

Delete a Page

  1. Locate the top of the page that you want to delete in the request form builder.

  2. Click the three-dot menu in the page's upper-right corner 1.

  3. Select Delete 2.

Reorder and Merge Pages

  1. Locate the top of the page to move or merge in the request form builder.

  2. Click the three-dot menu in the page's upper-right corner 1.

  3. Select Move up or Move down to change the page's order in the form 2.

  4. Select Merge with previous page to merge two pages into one 3.
