Request Forms - Don't List Unpopulated Fields if not Required

We want the non-required questions not to appear on the description if no answer is provided
 
 
In order to avoid extra text in the description if no information for that field is provided

Sad to see so few upvotes on this old request. I was just looking for a way to accomplish this request, and it doesn't appear to be possible at this time.

Hi Laura Boylan, thank you for bumping this thread. A potential workaround for this issue could be to redesign your request form so that instead of including the information in the description section, the replies could be mapped to, for example, custom fields.
 
Once the task has been created (upon request form submission), you can open it in the New Table view and simply hide the empty columns.
 
Could this be a viable option for you?
