Request Forms - Add Fields to Submit Page
Is there a way to add additional fields to the Submit/Contact page at the end of the Request Form process?
e.g. once the user has gone through the request form process they are automatically directed to the submit/contact page which only has Name and Email address on it.
I'd like to add phone number and company name to the submit/contact page.

Hi Kirk, welcome to the Community!
For External Requests, every submitter needs to fill in their email address and name (plus a security CAPTCHA. However, depending on how you create your form, you can ask for any additional information when creating the form and make the question 'Required' so they need to fill in an answer. Your admin can create a Request form or edit an existing one and add multiple custom questions which allows you to capture exactly what detail you need.

Thanks for getting back to me. What I'm looking to do is add to the email and name fields in the submit page. Rather than setting up a separate page before the submit page. That is, user would come to submit page and add their name, email address and the new fields like phone number and company name. Don't want to ask for the phone number and company name on one page and then ask for name and email address on the next.

Hey Kirk, I get what you mean, it makes sense and has been brought up before. The Team has been looking at adding enhancements in this area but it's not on their current short-term roadmap. It would be great to see your vote on Kevin's post here so the team can get a better understanding of how popular this request is.
I'll also be updating that thread when/if I have any new information about this feature request 👍
