New Automation Rule - Start/open new request form when custom field is updated

It would be amazing if we could automate the creation of a new Request based off a change to a custom field and point the automation to a specific request form.

My team's use case: there are several instances where if a PM updates a custom field value, they need to then go fill a request form out to start a related "project" with another team. There is currently no way to trigger the PM to do the request form (the closest I can get is to have it create a task for them with a link to the request form in question, but no way to then easily track whether they've actually gone and completed submitting that request form to our partner teams. 

Example: 

PM selects "yes" on a custom field for their project indicating that the project will require localization support. Process-wise, the PM is then responsible for submitting a request form for localization support form our Localization team. Would be great if them checking the "yes" on the custom field could automatically trigger the request form for them to populate immediately and submit to that team, rather than relying on a separate task or comment that is harder to track later on.
