Request Forms and Linked Databases

My company has a lot of acronyms and codes that people need to remember for various things. I was hoping to find a way to use databases in request forms to provide that information if people aren't sure which code to use.
For example, I would like to add a question like "which region is this for?" and have it linked to the region database. Once the requestor selects the correct region, it returns the region code so that the requestor can see it right there in the incomplete form and use it in other places while filling out the form (like in required naming conventions). Is this possible?
I know you can link the databases to custom fields, but that would require the requestor to complete the form, go look at the filled in custom field, and then manually update the request anywhere that code is required in the text. That wont work for us.

What I do for this is use the description part of the intake to link references they need to check before submission. That way can they can click and view and then go back to your form.
