Request form to Request Form
Use Case: I have built a Request Form (#1) for QAQC Data Requests. To process QAQC Data Requests, they must have the Material from another department (Material Request is Request Form #2).
When the planning to complete the test, they fill out a Custom Field with the Date they need the material by to do the request. The materials can only be checked out for 24 hrs.
The Title of the task the QAQC Data Request is something like "R1222" to signify sample number. Once the Need Material By date has been filled in, I am wanting this to add a NEW task into the Material Request folder, with the sample number and the due date that matches the Need Material By date.
I am currently set up to get notifications when the Need Material By date is filled in, then I create a manual task in another space for the Material Request. I am looking for a way to automate this, and open to other triggers, etc.
I would love some ideas or similar use cases.

you cannot clone task and its custom fields.
I've achieved something similar by adding to task an approval or add it to another space =)

I'm afraid that it is not possible to automatically create a new task with the specific sample number you mentioned.
