Require Comment on Status Change

I'd like to require that a user changing a status must add a note. This could look for a note posted in X minutes, or it could create a text prompt for a note when the status changes.

I'm really interested to hear more details - like what kind of info would a user add into this note?

A status change creates an update on the task stream, so you can see who changed the status and when. Or would you like to be able to add more details, e.g. reasons for the status change?

Insofar as to why, more details is exactly right. For example, if a task is set to a Deferred or Cancelled status, or any custom status that would indicate a change other than 'Still being worked on' or 'Done', it'd be nice to be able to prompt for a comment indicating why.

A more specific use case:
A task might be 'Current Step', meaning work CAN be done on that task, or 'Waiting' meaning we need a 3rd party to do something before we can move forward. If the task is set to Waiting, a manager will want to know what the task is waiting on. This status can change rapidly through a tasks life depending on a number of factors, so the specialist doesn't always consider that the reasoning isn't already outlined.

An important addendum to the suggestion, I would like to see a comment if a due date for a task changes as well. When something is set to 'Waiting', we'll usually set a date we expect to stop waiting, or a the very least, a date that we'll check in with the third party. Sometimes we check in, and discover we still need to wait. A note should be required alerting anyone checking the task as to what happened on that check in date, and why the new date was set.

In Short:
If the status or date is changed, anyone following that task may want to know why. Requiring a note will help us remove human error.
