Request Form: Mapping to Task

My company wants to start using the request form, but as I keep building I am experiencing gaps in the customization of the form. I would love to be able to map questions to specific tasks within a project. 

For example: In every project where there is a creation, we start by having a Kick-Off Meeting and then start the actual task after the kick-off meeting.

The way the project templates are build are: 
Project Name 
   Task 1: Kick-Off Meeting 
   Task 2: Start Project

I have a page in my request form that where the questions are specifically for the Kick-Off meeting, I would love to be able to map that question to my Kick-Off Meeting Task.
