Request form for current project

I am designing a form to track expense in a project. My idea is having a folder inside a project and some custom fields such as cost, category (hotel, transport, etc) and other internal fields. I would like this form would be standard for all my current projects. So, I expect to place my cursor inside a cost folder. Run the request form and create automatically a task with this information. I found a workaround by creating a dropbox with "parent folder assignation" but it is annoying to modify form each time I create new project. So, is there  a way to run a request form considering the project context where am I?

Would it be practical to flip your process so the request form drives the project launch?  Custom fields would be populated at the project level with this method instead of the task level.  Could that be beneficial for reporting purposes as well?
