Request Forms with Templated Tasks

I want to setup a workflow and request form for contract reviews so that if a client sends us a contract the salesperson can fill out a request form and it automatically routes it to the right party.

I want to have the request form have some subtasks to follow a process.  It would look like

•  Contract Review Request
• Executive Review
• Financial Review
• Account Exec Review

This way all (3) subtasks have to be checked as complete before the contract can be signed and returned.  They can happen in parallel (one doesn’t need to wait for the other to complete).

Is there a way to do this via a request form?  I was thinking about using the new project setup but I don’t want to create a new project each time.  More of a template of tasks.
