Connect custom field to custom item type

I would jump for joy if there could be a connection between custom fields and custom item types. Example, a user submits a form with the very specific dates required:

CF A = Due Date

CF B =  Due Date

CF C = Due Date

CF A = CIT A

CF B = CIT B

CF C = CIT C

This would allow:

- Multiple project dates entered into a form

- Control dependencies of sub items from CIT in blueprint from a form

- Only have to change the date in one spot (not in the CF for tracking and CIT for scheduling) (we have many data errors due to this)

That table would look like: 

                    CIT A | CIT B | CIT C
