Conditional Follow-Up Use Case

Reaching out to this community for help. I work on an internal communications team and we have a request form for users to request a communication to be sent. Here's my use case

  1. First, my users need to choose their audience - Managers and/or Associates.
  2. Second, they need to choose the communication channel(s) for each audience chosen in the first question - Managers have two channel options, Associates have five.
  3. Third, they need to answer questions for each channel chosen in the second question - e.g. publish date, message, etc.

The perfect solution would be a follow-up, on a follow-up, on a follow-up. But I know that's not possible. I'm open to the redirect to a new page, but that option isn't available for Multiple Answer questions, only Single Answer.

I'm looking for a solution that will only ask users for information about the channels they choose. Right now we have to present questions for all channels, even those they don't choose, which makes the form very long and confusing. Below is a user flow that illustrates what I'm trying to accomplish.

How would you recommend solving this?

Charlie Vincent would it be an option to reduce to one question?

One question for audience (or call it channel) Audience:

Associates - Channel A to Z

Associates - Amazon ...

...

Managers - The Weekly Brief?

Charlie Vincent it was just an idea for a solution. Do not make two questions. Just make one with in total 7 answer possibilities (combine audience and channel)
