The Product team is interested in implementing this suggestion, but there are currently no plans to add it to the short-term roadmap. If anything changes, I'll be sure to let you know. Thank you for all of your feedback!

Any updates from the Product Team on this one? It's been awhile since we've heard anything and there are over 70 upvotes on this feature request. Adding this feature would go a long way on making forms more understandable to the submitters.

If the images are not viable, I understand, but the team should at least create something like an optimised Section Header instructions/guidelines/SLA's etc. The Section Header is not great for that purpose and the notes below the questions are not enough.

Adding my upvote and use case here. We have one particular form in which sales partners or product specialists must select from among many (almost 50) different designs and note what changes need to be made to the design in order to make it a custom design. It would be useful to include an image of each of these designs. The only other signifier used to denote which design is which is a 3-digit design code and some of them are very similar (i.e. design 020, 022, and 002, for example) and we don't want people to select the wrong design(s) by accident.

Would really love to see this feature.... We use forms to capture incoming engineering tasks and ask the Requester to complete a preliminary Risk Ranking to help prioritise tasks. The ranking is done making reference to our corporate Risk Matrix...  The ability to insert an image would make it so easy.... as a matrix is the sort of thing that is really har to explain in words...

Adding my support for this. I'm a bit surprised that it's not in the short term roadmap given the amount of people looking for this. An external form only has the tiny company logo in the top left, but would look so much sharper if we had some ability to add and format additional imagery like a logo or background.

Lisa Any update on this? Looks like the community has been asking for this for 6 years. With as much as we all work with visuals and utilize forms, I'm not understand why this functionality has not been added. 

Lisa & Elaine Adding images would be great. However, my team would love to have the ability to add a link to a PDF with instructions and examples for our request form as an in between step. This seems like it should be an easy thing to add to requests since you can already add links and files in tasks. 

In the meantime, is it possible to at least add basic font editing abilities to the form? Bold, underline, italicize, and color? This would at least allow us to make important information stand out in the form. People tend to gloss over additional information when all the text is the same.

Adding a logo to a form that will be submitted to my business would be awsome.  The forms look so flat, and it would be nice to customize a little bit.

I last commented and added my use case for this wish-list feature in 2021 and since then I've discovered other potential uses for a feature such as this. I would love to be able to add a thumbnail illustrating which form is which, or what the outcome of each form would create, so that people always select the correct form. Also, within each form, I'd like to be able to add thumbnails to single-select dropdown lists so that users know they're selecting the correct option (a font, a color, or a tiny design template, for example). I wish I could add another upvote to the one I already added here in 2021! Please fast-track this feature -- as they say, "a picture is worth a thousand words!"
