Prefix Issue on Automated Request Form

Have a request form which is set to pull the prefix for the project by pulling from the result of which month the ed cal is for (Oct, Nov, Dec, etc) but when using the form instead of adding the prefix to the front of the project it gives me a string of numbers followed by a colon like 576306026: 

Note: the individual tasks are displaying the prefix correctly. 

Anyone have insight as to why this is happening?

Let me start by noting that the option to add a prefix will only appear when your request form is set to duplicate a task or project. 

If your request form is set to duplicate a project, then the prefix that is added from the answer to the selected question will only be added to tasks, subprojects, and subfolders. The prefix is not added to the title of the project itself.

Please take a look at a screenshot of what you will see after selecting to duplicate a project and hovering your mouse over the "?":

If your request form is set to duplicate a task, then the prefix will be added to all subtasks, but not to the title of the task itself. This is what you will see after selecting to duplicate a task and hovering your mouse over the "?":

With regards to your request form creating a project with a title that starts with a string of numbers followed by a colon, this is the default way that requests name the items that are created. If there is no question in the request form that maps to "Title", then the project or task that is created will be titled with the project/task's ID number/Job number (https://help.wrike.com/hc/en-us/articles/209603329-Job-Numbers) followed by a colon and the name of the request form.
