How do I create a new request form?

First, you need Admin access. If you don't have that level of access you won't be able to create the forms.

Next step, click on your profile image in the upper right corner of the screen/window. Choose Account Management. 

On the left side of the screen, you'll see a button for Request Forms. Click it and you are on your way. 

if you don't see this, then you don't have Admin or Account Owner access.
