Request Form Functionality Request: image choice checkbox/radio button
As part of our request forms, we would benefit HUGELY from being able to add a checkbox or radio button field where the choices are images instead of only text. We produce a lot of giveaways to our regional offices and that's how they're used to making their choices.

I concur. We definitely need a radio button option for the web form. We are unable to use the form as we intended because the checkbox option allows a user to select multiple options and we need several questions to be one and only one option. Some customers still select multiple options even though we've added (Select one and only one option) verbiage to the web form setup.

Agreed, the ability to upload images that users can select using checkboxes/radio buttons would be very beneficial.
I second Erin Arnold, can we at least have an option to only 'select one' if using the checkboxes? Yes/No questions look better with checkboxes/radio buttons then a drop down list IMO... please! :)
