Request Form - Dropdown Logic Issues

I have a multi-page request form created that is intended to drive our internal process. It is a 5-page request form. At the top of each page is a drop down question which has a "Yes" or "No" answers for the user to select. Depending on their answer, they either have to fill out the page they are on, or will be directed to the next page. 

As I'm testing the forms and selecting the answer that should put me on the next page, nothing happens. It's like it's not 'triggering' the code to move forward. 

Anyone have insight? Maybe I'm overlooking something in setup?

Hi Eric, thanks for your post and welcome to the Community! Let me show you how the Request form redirects look in some examples below.
 
The 'Yes' or 'No' dropdown question you mentioned should be on its own page and then any follow-up questions on a separate page. Depending on which answer ('Yes' or 'No') someone selects, they're redirected to a page you've created for each of the drop-down outcomes. See the GIF below. Note how each question redirects to a different page depending on the answer someone selects. 
 
 

 
Once you split the questions out like this they will display in your Request form for anyone who fills it in - as you can see below.
