Issue - space request form showing up for everyone

I'm creating a "space only" request form. However, it's showing up when my team goes to the green circle with the "+" symbol for them to be able to use, as well. I thought request forms created in the space is only accessible to those who have access to that space. Is that not accurate? Screenshots below. 

If not, can I adjust it to make the request form private to that space?
