Request Form - Field visibility option

It would be useful to have a visibility option at the field level, where the visibility of a field would be conditional to options selected in other fields.

Logic example, If user Q1=A1 + Q2=B3 -­> Show Q5

It would allow us to capture more info in the form and avoid creating extra pages for every detail we need to capture in the intake form.

I've been hoping for a feature like this for over a year now and it would be highly beneficial to both our requestors as well as my team for reporting purposes.

An if-then capability like this would be an important improvement to the request forms.

Really surprised this doesn't have more votes guys. The product team is thinking about different ways to add conditions to Request Forms. I'll be sure that this is brought to their attention during that period and report back here with info as I have it 👍

This would really be a great feature!

The only way it works right now is by putting all conditional questions on seperate pages, but that gets really confusing really fast if you have complicated forms and really isn't good form design. And I do not think that the example used above, where the visibility of one question is dependent on more than one other question is possible at all.

So many other online forms and surveys offer this type of functionality. I feel like it's what my team expects since they see it in most other places; it's the thing my team asks for every time we setup a new form: "Can this question only show up if the previous answer is ____" And I always have to say not unless it goes to a totally separate page, which most of the time they feel like is too confusing and time consuming to fill the form out. Especially since on more complicated forms you may have to answer several other questions before clicking next, and then the complaint is all of the relevant questions aren't grouped together because of all the different pages needed. It's really not a great user experience.
