Request Forms - Add Fields to Submit Page

Is there a way to add additional fields to the Submit/Contact page at the end of the Request Form process? 

e.g.  once the user has gone through the request form process they are automatically directed to the submit/contact page which only has Name and Email address on it. 

I'd like to add phone number and company name to the submit/contact page.
