Difference between Changes Changes Required and Rejected

Can someone tell me why sometimes when a job is being proofed there is a "Changes Required" button and sometimes there is a "Rejected" button? We HATE the "Rejected" button; it completely disrupts our workflow. Are we inadvertently turning this on somehow?

I know that sometimes REJECTED comes from not selecting "Approve files only." We always select that option because otherwise, it's like it wants you to approve the task itself. That might make sense for some teams, but not for us.

Thanks. I see the Rejected above the files. What I'm talking about is a rejected button inside the proof where you make your comments.
