Use multiple questions to populate the request form prefix
In request forms, we can combine fields to create a very descriptive title.
However, the "Add Prefix" feature can only reference 1 custom field.
It would be helpful to combine multiple custom fields in the prefix as well.
Our current workaround is to add a question (called 'Prefix') and have the requester manually enter the two fields (not ideal).

Hi this might help with your work around to not have to manually add
In the form, for any questions that needs to be apart of the title make sure "Add answer to" = Title (+ any field the info needs to be in)
Then based on the order of the question(s) that points to title will dictate which field is first to last as a "prefix" to the title, just make sure the last question that belongs as the actual title, is last.
Q1 Q2 Q3 Q4 = Prefix A - Prefix B - Prefix C - Title
Q1 Q3 Q2 Q4 = Prefix A - Prefix C - Prefix B - Title
The only down side is all the dashes

Michael Hebert - that works great for the title. I'm looking to add a prefix to all the tasks created from the request form.
The title (highlighted in green) has the customer - part number - revision. The yellow is the "prefix" which I'd like to be the same (customer-part number-revision), but in the picture below it's only the part number.
As far as I can tell, the request form can only select 1 field to be added as a prefix for the tasks. Unless there's another workaround of combining fields in a form.
