Adding Fixed text instructions in request forms

I am trying to add an instruction section to my request form. Among the field that the user is filling in when using the request, I would like to be able to show a small table with some instructions.

Thanks for the question! You can use the description field under the title to add instructions for your request forms.
  
It will appear on the form under the header, and also in the list of request forms.

We also use the "helper" text to add very specific direction - it does not appear there is a character limit - so you can copy paste a full block of text if needed
