Give details when error icon shows up on request form

Hi team, 

We've noticed that there are caution icons that appear to indicate an error in request form settings, but there is 0 indication of what the error is.

It's appreciated that issues are flagged, but without detail or direction, we cannot possibly understand what the issue is and how we can fix it. 

It would be great if there was a piece of text that appears when you click or hover over the issue, or if it would just incorporate the text within the UI near the icon without needing to interact with it.

Thanks for posting this. I'm curious about the configuration where you're only seeing the error icon without any text explanation. Could you share a bit more about that? I'm also raising a ticket with our support team to look into it. Thank you!
