Request Form Breakout Logic

Hello, 

We love the breakout logic that will do a follow up question in a request form. Is this possible to do a couple of times? More than once would be helpful for some of our questions in our intake form and how we start thinking of a more unified intake form.

Glad to hear you find the conditional follow-up question option on request forms useful! Currently, you can only add one follow-up question to a single question in a form.
 
However, as a workaround, you can use the Multipage Request Forms option. This will let you redirect users to different pages based on their answers, effectively adding multiple follow-up questions.
