Comments not showing up where they were placed!

Hi all,

We've had an incident where comments (whether highlighted text or just dot w/text box) are showing up shifted erratically across the page. You can read the comment, but the highlight is not where it's supposed to be so there is not context. I tried refreshing, restarting, didn't work. When I export the PDF with comments they show up in the right place. 

Any ideas?
