Can you add a Duration to a Form? Instead of having to enter a Start and End Date I would like to set an auto duration for the new Task based on the submission date. Submission Date = Start Date and auto duration automatically calculates and enters the End Date.

You can already do part of that. You can use a blueprint or template task in request form and have duration set there. That way you only have to have one question regarding dates in the form itself (don't forget to setup "align with start date or end date"). Option to have Start date to be equal to submission date would be excellent thou.
