Great idea! Request forms are for sure one of Wrikes best features and this will add a lot! Especially now when you are also able to communicate with the requester.

A really great suggestion! The name of the former employee could just be greyed out instead or put "Deleted user" inside ().
Use the menu button, the 9 squares menu. There you have access to "My-to-do" from everywhere in Wrike!
