Ticket ID & Custom Subject on Request Forms?

Great question. My workaround for this is to have check boxes in my Request form, that when checked, become a part of the Title. For instance, i have a form that starts a task to reinstall some software. The first question is "What store #", I have that be part of the title. Next question is "today's date" which becomes the assigned day of the task to be created. I then have one checkbox available: Install new software, which I have placed in the title. The resulting task created becomes "## - Install new software", with the ## being the store number. This is just a basic request form, but saves time and issues as to not having to duplicate a task.
